The Contact Report Lookup is used to search for contact reports that have been recorded for trip notes, telephone calls, and other types of communication with entities, suspects and prospects.

Note: If you are performing a template lookup, some of the fields described below may not appear on the form.
Data Fields:

Contacted |
The Advance entity ID of the contact that you wish to look up. |

Name |
The name of the contact you wish to look up. |
|
Contact Type |
The type of contact report you wish to look up. Values in this drop-down list are validated against the tms_contact_rpt_type table (V1). |

Purpose |
The purpose of the contact report you wish to look up. Values in this drop-down list are validated against the tms_contact_rpt_purpose table (V2). |
|
Unit |
This code is used to identify the school or unit. Values in this drop-down list are validated against tms_unit_code (UU). |
|
Program |
The program of the contact report you wish to look up. Values in this drop-down list are validated against the tms_program table (TB). |

Author |
The Advance entity ID number of the author of the contact report. |
|
Joint |
This field indicates whether you wish to look up joint contact reports. Hard-coded values include Joint, Other, and ALL. |

2nd ID |
The Advance entity ID number of the joint or other entity involved in this contact report. |
|
Contact Date |
The contact date for the contact report that you wish to look up. |
|
Initiated By |
The party who initiated the contact for the report you wish to look up. Values in this drop-down list are validated against the tms_contact_initiated_by table (UE). |

Outcome |
The outcome of the contact for the report you wish to look up. Values in this drop-down list are validated against the tms_contact_outcome table (UD). |

Attitude |
The attitude of the contact for the report that you wish to look up. Values in this drop-down list are validated against the tms_contact_attitude table (UB). |

A brief description of the contact report you wish to look up. |

Rep ID |
The Advance ID number of the representative with whom the author met. This field and the Rep Name field are mutually exclusive, meaning that you may enter in information in one of the two fields but not both of them. |

Rep Name |
The name of the representative for the contact report you wish to look up. This field and the Rep ID field are mutually exclusive, meaning that you may enter in information in one of the two fields but not both of them. |

Rep Type |
The representative type of the contact report you wish to look up. Values in this drop-down list are validated against the tms_rep_type table (W1). |

Text associated with the contact report you wish to lookup. By default, the logical operator will be set to ‘Contains’. |
