The ID number of the entity holding the membership (individual or organization) that you wish to look up. |

The alternate membership ID number you wish to look up. |

Club Code |
The code of the club to be connected to the membership for the entity you wish to look up. Values in this drop-down list are validated against the tms_memb_club table (MA). |

Rate Code |
The rate code (dues) for the club you wish to look up. Values in this drop-down list are validated against the tms_memb_rate table (M3). |
